We are a peer run/peer driven agency who has multiple funding sources. We offer a multitude of services from Health Home Care Management, Advocacy, Warm Line, School Based Services and more.

About Home-Based Crisis Intervention (HBCI)

Home-Based Crisis Intervention (HBCI) is a short-term, intensive program designed to help children/adolescents who are at risk of an out-of-home placement and/or are experiencing repeated hospitalizations and/or hospital emergency department visits related to behavioral health challenges.  

Interventionists work with the children and their families to produce safety plans, assess for overall needs, make connections with needed resources, build and use coping strategies, and coordinate care to bring about stabilization in the home and community.  

Children/adolescents must be between the ages of 5 years and 20 years and 11 months.  

Referrals are accepted by email: hbci@communityconnectionsfcny.org and/or through the 24/7 support line: (518) 481-8582.

Participation in the program does include at least one home visit and discussions with caregivers as well as the children/adolescents.  There are no financial eligibility requirements.  Acceptance into the program is contingent on program openings and intensity of need.
