HEARTT of Clinton County Purpose: Collectively understanding the need to strengthen the Children and Families Service Delivery System, Clinton County’s community-based services came together as the HEARTT coalition.

In 2021, this collaborative effort set an intention to decrease service delivery gaps and collaborate on data collection for the children and family service delivery system. The HEARTT Coalition agreed to convene to better serve and strengthen the current Children and Families Service infrastructure. The HEARRT of Clinton County is for the convening of stakeholders and partners that create the framework for the Clinton County Systems of Care (SOC). The Systems of Care framework is a coordinated network of services and supports organized to meet the physical, mental, social, emotional, educational and developmental needs of children and their families. The HEARTT of Clinton County put forth the following Vision and Mission Statement: HEARTT of Clinton County Vision Statement: Working together, we will create a community legacy of healing, resilience, and transformation through leadership, collaboration, training and education. HEARTT of Clinton County Mission Statement: Empowering individuals and families on their journey toward wellness and recovery by strengthening social, emotional and physical health resources in our community.
